This version runs locally for development and testing. Use synthetic details only. The clinic must finalise its privacy notice before public launch.
What the form stores
The form stores your name, email, phone, pet name and species, visit reason, preferred date and time of day, optional message, and acknowledgement of data use for the request. Only authorised staff can access requests in the administration area.
Communication and retention
Requests are stored in this local installation. No automatic emails or text messages are sent. The clinic must define retention periods, the deletion process and responsible contact details before public operation.
Cookies and external links
No analytics or advertising trackers have been added. WordPress uses administrator login cookies and Polylang may remember language preferences. Google Maps opens only when you follow its link. Future externally hosted videos should load only after your choice.
